PACB CE Program Renewal Attestation Form

 

For Single Program Provider Continuing Education Approvals

 

PACB permits one renewal of an approved CE program when the content has not changed. Renewal eligibility depends on the original delivery format.

  • Live-only programs may be approved for one year and renewed once for one additional year.
  • Recorded or on-demand programs may be approved for two years and renewed once for one additional two-year period.
  • A program that has already received its one permitted renewal is not eligible for further renewal.
  • A program with changed content, title, speaker, learning objectives, CE hours, delivery format, or materials must be submitted as a new CE application.

 

Incomplete forms, forms without a prior PACB approval code, or requests for programs that have already received their one permitted renewal will not be reviewed as renewal requests.

A separate form must be submitted for each program.

  1. The program identified above is the same program previously approved by PACB.
  2. The title, content, learning objectives, CE hours, presenter qualifications, and delivery format have not changed, except as disclosed on this form.
  3. I understand that PACB allows only one renewal per approved program.
  4. I understand that if the program has materially changed, PACB may require a new CE application rather than a renewal.
  5. I understand that PACB may request supporting documentation before approving the renewal.
